GLP-1 denials (Wegovy, Ozempic, Zepbound): what actually flips them
BMI thresholds, T2D pathways, formulary exceptions. The three angles that win most GLP-1 appeals — and the one that wins them faster.
GLP-1 weight-loss drugs are the most-denied prescription category in U.S. healthcare right now. Wegovy, Zepbound, and Mounjaro lead the pack. The denials look identical across carriers — 'not medically necessary,' 'BMI threshold not met,' 'lifestyle modifications first' — but the path to reversal varies.
Three angles work consistently. We use whichever one fits your case.
Angle 1: The T2D pathway. If you have type 2 diabetes (or even prediabetes with elevated A1C), the GLP-1 has a separate coverage track on most plans. Ozempic and Mounjaro are FDA-approved for diabetes; Wegovy and Zepbound for weight loss. If your plan denies the weight-loss indication but covers the diabetes one, switching the prescription path to the molecule's diabetes-approved name is often a 14-day fix.
Angle 2: The medical-necessity letter. Almost no GLP-1 denial cites the actual clinical criteria the carrier used. By law, you can request them in writing. Once you have them, your prescribing doctor writes a letter addressing each criterion specifically — comorbidities, BMI history, prior trial of lifestyle interventions, family history of cardiovascular disease. We draft the letter; the doctor signs it.
Angle 3: The formulary exception. If the drug is non-formulary on your plan (a different category than 'denied'), the manufacturer has clinical justification packets pre-built. Novo Nordisk and Eli Lilly both publish them. Filing a formulary exception with the manufacturer's packet attached is the fastest path on Aetna and several BlueCross variants.
Speed matters. GLP-1 prescriptions usually have a fill window. Carriers know this — drag-out is part of the strategy. Filing within 5 days, with the right angle attached, beats filing in week three with the wrong angle.
