What to do, in order

  1. Step 1

    Ask for urgent handling

    If interruption risks rejection, infection, hospitalization, or graft loss, state that in the request and attach the transplant clinician's statement.

  2. Step 2

    Separate formulary from medical benefit

    Some transplant drugs run through pharmacy benefit, others through medical benefit depending on setting and payer.

  3. Step 3

    Get the transplant team's letter

    It should explain diagnosis, transplant date, current regimen, why substitution is unsafe or inappropriate, and consequences of delay.

  4. Step 4

    Ask for bridge supply

    Request temporary or transition supply while the exception or appeal is pending.

The deadline that applies

Urgent appeals must be handled on the urgent timeframe when delay could seriously jeopardize health. ACA drug exceptions can be expedited within 24 hours; Part D exceptions can be expedited under 42 CFR 423.578. The denial letter controls the formal appeal deadline.
