How do I prove my insurer received my appeal?
Keep the exact appeal package, its submission confirmation, and the insurer’s acknowledgment or appeal reference number together. A delivery receipt helps document transmission, but you should also check that the correct appeal department received the complete package and logged it against the right claim.
What to do, in order
- Step 1
Confirm the correct destination
Use the appeal instructions for your particular plan and decision. A general customer-service address or provider billing inbox may not be the appeal destination. Note where you found the instructions.
- Step 2
Save an exact copy of the package
Give the final file a clear version name. List the attachments and page count so you can explain what was included. Keep the draft separate from the version you actually submitted.
- Step 3
Save the confirmation for the accepted channel
For a portal, save the confirmation screen and upload list. For fax, keep the transmission report and destination number. For mail, keep the tracking or delivery record alongside the document list. Use only channels the plan accepts.
- Step 4
Check receipt and record the response
Ask which appeal reference number was assigned, what receipt date was logged, and whether any pages or authorization are missing. If the plan cannot locate it, ask for the next step and document both the original attempt and any resubmission.
