Durable medical equipment was denied. How do I appeal?
DME denials usually say the item is not medically necessary, not primarily medical in nature, not the least costly alternative, or missing documentation. The appeal needs the clinician's order, medical records showing functional need, the plan's DME criteria, and a supplier quote or code that matches the requested item.
What to do, in order
- Step 1
Identify the DME reason
A wheelchair, CPAP, brace, hospital bed, pump, or prosthetic can be denied for different reasons. Read whether the plan disputes the diagnosis, item type, documentation, code, supplier, or benefit.

Get the policy and code
Ask the supplier for the HCPCS code and the plan for the DME policy applied to that code.
- Step 3
Have the clinician document functional need
The record should explain what the patient cannot do without the item, what alternatives failed or are unsafe, and why this specific model or feature is required.
- Step 4
Correct coding before appealing if that is the issue
If the supplier used the wrong code or left out modifiers, a corrected claim may be faster than a medical appeal. Preserve the appeal deadline anyway.

At least 180 days to appeal on most employer and ACA plans. Medicare and Medicaid DME appeals use their own ladders and notices; the date printed on the DME denial controls.
