Xtandi (enzalutamide): Insurance Coverage and Denials

Xtandi is an oral targeted therapy classified as an androgen receptor inhibitor. It blocks androgen signaling at several points, competing with hormones such as testosterone to prevent activation of the androgen receptor that drives prostate cancer growth. The FDA has approved Xtandi for prostate cancer across several settings, including castration-resistant disease, metastatic castration-sensitive disease, and non-metastatic castration-sensitive disease with biochemical recurrence at high risk for metastasis.

Insurers commonly require prior authorization and often verify the specific prostate cancer setting, prior or concurrent androgen deprivation therapy, and disease status.
