- 2Upload or photograph the denial letter. A reviewer reads it and tells you in writing whether it can be appealed.
- 3If it can, Apellica prepares and files the appeal. Nothing upfront. 10% of what is recovered, only if it succeeds.
Fewer than 1 in 100 denied claims is ever appealed. About a third of appeals to the insurer succeed, and independent reviewers side with the patient more than half the time (KFF, 2024 claims; California DMHC). Your plan must give you its reasons and the criteria it used, and you have a deadline printed on the letter.
